We serve adults aged 18 and up with an ADHD diagnosis or with ADHD symptoms. Our care team (therapists and coaches) is spread across the United States.
The process starts with individually completing a clinical survey. This first step allows your therapist to get a basic understanding of your symptoms, and guide their following sessions with you. The survey is completed through the Agave Health app.
Once the survey is completed, you can schedule your first appointment with your therapist.
You can choose to schedule the following two appointments right away, or do so after each appointment. The appointments cannot happen within a single day, however they do not need to be spaced weekly.
The third and last session of the Evaluation and Diagnosis is a wrap up session where your therapist will both summarize their findings, as well as recommend a care plan adapted to your needs.
Before your initial appointment, you are required to complete the self-reported survey.
If you struggle to complete it and are a member of Agave Health, you can seek the help of your Agave Health Coach to help you complete it.
Yes, our licensed therapists can conduct an ADHD evaluation and provide you with a diagnosis. Our therapists always provide you with a recommended care plan alongside this diagnosis, whether it is positive for ADHD or not.

Yes. By purchasing an Agave Health subscription, you access all our care modalities, which include therapy, coaching, self-learning and more. If your employer sponsors your Agave Health subscription, you can insert your company code upon purchase to access all our care modalities. We do provide ADHD-informed therapies to treat depression, anxiety, and sleep challenges that co-occur with ADHD. Please note that our program is not able to treat individuals who also present self-harm behaviors, suicidality or suicidal ideation. If these are a primary concern, contact your PCP or care team for recommendations on resources and treatments for these conditions.
If you do not meet the ADHD diagnosis threshold, but executive function challenges are a core concern, the Agave Health team can support you
Yes. Our clinical services are conveniently provided exclusively online via HIPAA-secure telehealth technology (i.e. video visits and chat).
We select licensed clinicians with experience in delivering ADHD treatment. After joining our team, all clinicians receive ongoing training from leading ADHD experts to deliver best-in-class ADHD-informed therapy, including evaluation and diagnosis.
Yes. Upon indicating your state of residence, you can choose among the Agave Health licensed clinicians available in your state.
Outside of sessions, you can write messages to your therapist through the Agave Health app.
In addition to licensed therapists, Agave Health provides you access to ADHD Coaches, as well as online resources. The Coaching, support and self-serve services are all accessible through the Agave Health subscription.
The Agave Health subscription is purchased separately from the Evaluation and Diagnosis service. Some employers sponsor Agave Health subscriptions.
The Evaluation and Diagnosis package costs a total of $650. This price includes the individual completion of a clinical survey, its analysis by your therapist, two 45-minutes synchronous virtual sessions with your therapist, as well as one 30-minutes synchronous session focused on providing your with your evaluation’s outcome and a recommended treatment path.
Therapy services (including the Evaluation and Diagnosis service) are currently available in New York, California, Florida, Pennsylvania, Wisconsin and Iowa. We continuously add more states and update this list accordingly.
We currently accept most health insurances in FL and PA. We continuously work to accept insurances in more states and update this answer as our list of accepted insurances grows. If you are not located in one of the covered states, we provide you with a superbill at the end of your Evaluation and Diagnosis process to support out-of-network claims or FSA reimbursement.
The services included in the Evaluation and Diagnosis package are covered by CPT codes 90791 and 90834.
For the Evaluation and Diagnosis service, the credit card on file will be charged for the entire package upon initial purchase. For all other therapy services, the credit card on file will be charged following each appointment/session. After the credit card is charged, you will receive an email receipt.
We understand that sometimes plans change. Please provide your therapist with at least 24 hour notice if you need to cancel or reschedule an appointment. Late cancellations or missed appointments may result in a cancellation fee of $50.
Yes. You will receive by email a superbill at the end of your Evaluation and Diagnosis process to support out-of-network claims or FSA reimbursement.
No. By purchasing the Evaluation and Diagnosis service, you do not commit to anything else. You can choose to purchase a subscription to Agave Health separately at any time.
